Vaccine Information Statement
Vaccine Information Statement is a formal description of a vaccine, with a concise description of the benefits of the vaccine, a concise description of the risks associated with the vaccine, a statement of the availability of the National Vaccine Injury Compensation Program, and is required as a provision of the United States National Childhood Vaccine Injury Act. Such materials shall be provided prior to the administration of a vaccine set forth in the Vaccine Injury Table.
