Mayhem Festival 2012 was the fifth annual Mayhem Festival. Dates were announced on October 26, 2011.[1]

Mayhem Festival concert chronology

On June 7, 2012 Mayhem officially announced that High on Fire had pulled out of the festival, due to lead singer Matt Pike entering treatment for alcohol rehabilitation.[2][3]
