The division was formed for the 2000-01 when the old Midlands 4 East was split into two regional divisions called Midlands 4 East (North) and Midlands 4 East (South) and gained its current title for the 2009-10 season due to further restructuring to the Midlands League. Promoted teams tend to move up to Midlands 2 East (North) while demoted teams typically drop to Midlands 4 East (North).

Notes
- Mistakenly referred to as Midlands 4 East (North) on RFU website.
- Glossop, Vipers, Loughborough were also promoted. The reason for more promotion than usual was due to Midlands league restructuring. Long Eaton, Glossop and Loughborough went up to Midlands 3 East (North) where they joined champions Market Bosworth while Vipers went up to Midlands 3 East (South)
- There was no relegation due to restructuring of the Midlands league with teams instead being transferred to the new look Midlands 4 East (North) or Midlands 4 East (South) for the following season.
- No relegation as the division would be increased from 10 to 12 teams for the following season.
- No relegation as the division would be increased from 11 to 12 teams for the following season.
- Last season as Midlands 4 East (North). Would be renamed Midlands 3 East (North) for the following season.
- Luton's title was won when division was unified as Midlands 4 East.
- Market Bosworth's title was won when division was unified as Midlands 4 East.
