How is TSC (TKIP sequence counter) calculated, in WPA?

I'm reading about 802.11, from IEEE's PDF, from page 1191, and in particularly I'm reading about TKIP. To decrypt and encrypt, you use a TSC (TKIP Sequence Counter) as you can see in these images: How, and from what, is the TSC calculated?

WPA2 and ChopChop attack

I know that WEP is vulnerable to the ChopChop attack. Could the ChopChop attack be carried against WPA2-PSK as well? Some Internet articles suggest this possibility, I want to be sure though.

TKIP key length and initialization vector size

It's not clear to me from reading the wiki if TKIP encryption uses a 128 bit RC4 encryption key and a 48 bit IV, or if the 128 bit is the total, and the actual key size is 128 - 48 = 80 bit.
