Killsplosion. When the pain elemental reaches 0 hit points, it explodes and releases lost souls. The explosion deals 5d6 fire damage to all creatures within 10 feet. A successful DC 13 Dexterity throw halves the damage. 1d3 lost souls appear in the space that the pain elemental occupied.

ACTIONS

Claw. Melee Weapon Attack: +4, reach 5 ft., one target. Hit: 4 (1d4 + 2) slashing damage.

Spawn Lost Soul (Recharge 4-6). Ranged Weapon Attack: +6, reach 30 ft., one target. Hit: 9 (2d4 + 4) piercing damage and 10 (3d6) fire damage. If there are no more than six lost souls within 60 feet of the pain elemental, a lost soul appears in a space within 5 feet of the target.


A pain elemental is similar to a cacodemon: a large, floating orb-shaped monsters with leathery skin, wide sharp-toothed maw, a pair of horns, and a single large red eye. It has a pair of stubby, three-clawed arms.

A pain elemental gathers and consumes lost souls, torturing them in its fiery interior.

Variant: Rahovart

A rahovart is an advanced pain elemental with no maw, but has smaller mouths in place of each claw. It has a vibrant blue eye and a black horse-like mane. Make the following changes:

  • It looses its claw attack.
  • When it uses the spawn lost soul action, it can make the attack twice.
  • It has 85 (9d10 + 36) hit points.
  • It has a CR of 5.
