Evasion. When the lost soul is subjected to an effect that allows it to make a Dexterity saving throw to take only half damage, it instead takes no damage if it succeeds on the saving throw, and only half damage if it fails.

ACTIONS

Bite. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 8 (2d4 + 3) piercing damage and 7 (2d6) fire damage.

Screaming Charge (Recharge 5-6). The lost soul moves up to 30 feet without provoking opportunity attacks and makes a bite attack. If it hits, it deals an additional 3 (1d6) fire damage.


Lost souls are tortured victims of Hell that have been weaponized by demons. They resemble floating, flaming skulls. They float aimlessly in silent anguish until they see a living creature. They can attack from some distance away if they move and then perform a screaming charge. They are usually encountered in groups.
