Mythic
Epic tier of 5th edition D&D consists of 20th-level characters engaging in CR 20 to CR 30 encounters, and receiving epic boons (or similar) as rewards.
What I call the "Mythic" tier is the step beyond that. Players have many epic boons and can be considered demi-gods, and deal with threats from the extremes of the multiverse: objects from the dawn of time, sanity-blasting oddities from the Far Realm, awful creatures from the farthest-flung dimensions.

Mythic Equipment
There are two new rarities of magic item, epic and mythic. The DM should only place them as loot, or allow them to be crafted, if the PCs are 20th level. Although these items are more powerful than legendary items, they are not as complex or as narratively important as artifacts. Their function is to better equip the players against high-CR challenges.
Downtime Activities
Crafting an epic or mythic magic item requires truly extraordinary materials.
Selling a magic item in a mythic campaign assumes that the seller can find buyers more exotic than the usual material realm cityfolk. Often only extra-planar entities have the capability of purchasing such items.
