Heine Rammsteiner
The main protagonist. More or less. Not completely right in the head.

- We eventually learn why: Einsturzen noticed Heine's exceptional abilities and wanted to experiment on him and attach the "Master" spine to his body. Heine accepts under the hope that with it, he'll be stronger and better able to help his friends escape. Unfortunately, once the spine begins to successfully fuse to his body, Einsturzen decides that she doesn't need the other test subjects anymore and has them kill each other. Heine escapes the "Master" process early but not enough to stop himself from disemboweling Lily in cold blood.

A private investigator slash occasional hired gun who sometimes teams up with Heine. Loves his ciggies something good. Missing an eye.

Naoto Fuyumine
A swordswoman wandering the city trying to find information on her parents' killer. "Naoto Fuyumine" is not her birth name.

- Action Girl: Easily capable of fighting on the same level as Heine.
- Anti-Heroine: Type III-IV.
- Badass Normal
- Berserk Button: Mentioning the other "Naoto," and badmouthing the man Fuyumine are a good way to get her riled.

- The Chick: She's the only main character who doesn't think Murder Is the Best Solution. Maiming people's alright though.

Mihai Mihaeroff
A retired assassin and occasional errand runner for the owner of Buono Viaggio. He gets lost. Often.

Angelika Einsturzen
A Mad Scientist working for an as of yet unnamed underground organization.Giovanni and Heine occasionally call her "Mother."
- Abusive Parents: To the Cerebus spine children, most of whom are probably not her actual children. She makes them fight huge monstrosities to earn her fleeting love.
- Affably Evil: When she first makes an appearance in the manga, she comes across as slightly matronly if weird. And then the madness shows itself.
- Ax Crazy: She has yet to pick up an actual weapon but someone who smiles while people are getting mauled barehanded is not right in the head.
- Back from the Dead: The original body is killed (or at least one of them is) by Heine. Another body walks up and keep talking to Heine and Magato as if nothing happened.
- Blondes Are Evil
- Body Surf: She's a Cerebus spine person too but instead of healing, she can just transfer her consciousness into another body. After Heine shoots the first one to death, her next body is Lily.

Recently revealed to be the original Naoto & also the killer of Naoto Fuyumine's parents.
- Affably Evil: Rather polite and formal to Bishop, even leaving him her name.
- Badass Boast:"Forgive the lack of ceremony but I have to kill anyone who sees us. Any last prayers?"Bishop escapes and is blind, so technically this is still true.

- Vague Age: Or perhaps, Younger Than They Look. In the prequel, Magato seems noticebly older than Naoto (early 20's vs mid teens) whereas in the series proper, he seems 5 years older at the most.
- Wild Card: No one is sure who's side Magato is on. He seems a bit obsessed with killing Naoto but expresses something resembling fraternal concern when her fighting style becomes inexplicably more cutthroat. He also drops his fight with Heine over the suspicion that she's in danger. It's implied that the only thing Magato cares about in any intense manner is Naoto but for what reason, purpose or even if this is a good thing hasn't been explained yet.
