- Remember Pinoko, the Clingy Jealous Girl sidekick of Dr. Black Jack in the anime and manga of the same name? Well, would you believe that she's an Artificial Human constructed by the aforementioned doctor from a woman's underdeveloped parasitic twin and some spare parts while he was drunk? It's totally the truth, we swear.
- There's also an episode that deals with a love triangle between a young doctor, his fiance and - get this - a surgically altered, sapient deer. We have pictures if you don't believe us.
- There's a chapter in the manga in which Black Jack operates on his own intestines... in the middle of the Australian Outback... after being stranded for several days... while being attacked by dingoes... Yeah.
- Hey, It's That Voice!: Black Jack is voiced by Akio Ohtsuka, best known for as Solid Snake.
- And in flashbacks, you might recognize teenage Black Jack's voice better if he had hands that glowed with an awesome power instead of the hands of God.

- In the OAV dub, Black Jack is Jin, Hideo Kuze, and Hajime Saito; Pinoko is Hikaru and Arusu!
- Jo Carol Brane, the first movie's leading lady, is voiced by Mayo Suzukaze. Yes, THAT Mayo Suzukaze.
- Jossed: The second Kei story seems to have been written specifically to disabuse fans of the notion that Kei/Megumi had been physically transformed into a man by the removal of her ovaries and uterus.
- Life Imitates Art/ Reality Subtext: The manga story Tenacity is pretty heart-wrenching in light of the author's final fate. What makes it even worse it that the dying doctor's personal physician is "played" by Tezuka himself.
