• Critical Research Failure: Not by any of the members, but rather by Casey's doctor. Casey had a root canal while on tour and was prescribed an Vicodin for the pain. As it turned out, Vicodin didn't mix well with the antidepressants and anti-anxiety medication Casey was on and he died as a result. The doctor who prescribed Vicodin really should have checked whether Casey was on any medication before issuing the prescription. (In fact, this is supposed to be standard procedure.)
