PC often fails to wake up from sleep

Hope that's the right place to ask you about my issue. I'm having weird problem with my PC, namely, sometimes it won't wake up from sleep, fans are spinning, and my LCD wakes up from standby too, however, screen is black, there's no sound and computer doesn't react to keyboard, power button etc. I thought that maybe it's caused by Windows, maybe some driver related issue, but on Linux Mint I'm getting the same results, so I'm guessing it's hardware issue, however I've no clue what maybe causing it.. I'm facing this issue for a very long time, unfortunately, I can't recall whether or not the same issue was happening with another motherboard (but I'm sure it's not the PSU, because the same happens with two completely different models) and because it's totally random, it's very difficult for me to find the most probable cause. I hate it, because in 99% cases, after hard restart, I'm losing all my unsaved work, but what is interesting is that sometimes, very rarely, after such restart, at least Windows, was able to boot/wake up like nothing wrong happened with all my work/apps intact. Also sometimes, again, at least on Windows, during such failed wakeup I was able to see some glitches on the screen (permament, until hard reboot), but it's very rare. Also sometimes after some time in such void state the PC will shutdown on its own.

Do you know what maybe causing such weird behavior? Can it be some kind of damage to my hardware? Is there any way to trace what's preventing my PC from waking up normally? If it helps, my mobo is ASUS P7P55D with Radeon 5770 and i5-750. PC is not overclocked.
