Libreoffice base combo box populating text boxes

Question

I am using Libreoffice Base 4.2.8.2 on Zorin OS 9
I need a combo box (Business names) which will fill in text boxes text box 1 (Address)text box 2 (Phone) text box 3 (email) etc. It must be a combo box as I want to add new records as well as select current records.

Please assume I know nothing about base or programming.
