did my USB just die? Can it be fixed?

I bought a 64bg Sandisk Extreme two weeks ago and today it decided to just quit on me. When I plug it into my computer I can hear that Windows sees it but not in Windows Explorer. I can view the device in Device Manager after I do a refresh for Hardware Changes.

What I see in DiskPart:
