Hard Drive - Windows 8 Pro 100% Disk Usage Whenever Running Any Program

I have a AMD four core processor, a very old hard drive of very low capacity below 200GB and 2GB DDR3 RAM. I am facing a serious issue from the last 2 weeks. Before I had a old AMD processor which has very low clocked 2 cores. My computer hangs and freezes, so I think it is the issue of CPU and I got a new one 2 weeks ago. And installed new OS (same OS before also). But from the very first boot whenever I try to run any program even browser also. It takes time and in task manager it shows 100% disk usage. Is this the fault of my hard drive? I tried paging, closing all those three tasks Superfetch, Windows Search and Windows Update. Nothing happens, whats the issue my ram usage normally is 80% and the CPU usage is 12-16%. Don't know what to do? As this is the new and fresh install, I think it is fault of HDD.

12GB is WAY too little memory. I suspect you are HEAVILY swapping and all the cpu resources are being dedicated to swapping in and out memory regions to run some of your applications. – mdpc – 2014-10-23T06:08:55.370
